Instructions for completing SAMI application for membership:

Type of Membership

Select “Regular Membership” or “Junior Associate”. Junior Associate pays no dues and has no vote and is open for children under the age of 18.

Applicant Name(s) and Address:

Enter the name, address, telephone numbers and emails of all persons requesting membership. (For family membership, please list information for both persons)

Signature/Date

Signature(s) of person(s) requesting membership and date signed. For Junior Associate, the junior and one parent must sign.

Sponsor

Must have the signature of two members of the Samoyed Association of Madison, Inc, who are in good standing.

Dogs you own

List all dogs owned and by breed, name and AKC number (if applicable).

Dogs you are listed as the breeder

If you have bred any dogs, list the breed, whelping date and litter registration number. If none, write “None”.

Memberships in other dog clubs

List all memberships in other clubs regional and national, including any offices held and committee experience. If none, write “None”.

Dogs you have shown to championship level

List all dogs by breed, name and AKC number of any dogs you have shown to championship level. If none, write “None”.

Dogs you have shown to an obedience and/or working degree

List all dogs you have obtained an obedience title or working degree by breed, name and AKC number. If none, write “None”.
